Staff record what actually happened. Everything a funder asks for is calculated from that, so nobody retypes a number into a report.

Take attendance where the program happens

Youth sign in and out on a phone or tablet, or on a printed sheet your staff scan back in. Works in a gym with no signal and uploads when it reconnects.

Step 2

The session is signed and locked

Your instructor certifies it, a supervisor approves it, and the record locks. Corrections make a new version, and the original stays exactly as it was signed.

Step 3

The packet assembles itself

Pick the grant and the reporting period. Every deliverable in your agreement comes back answered, measured against its minimum, with the sign-in sheets and signatures attached.

Attendance that holds up

Individual sign-in and sign-out times per youth, not a headcount. Days attended, hours served and unduplicated counts come out of the same records.

Units of service, your funder's definition

One funder counts hours, another counts sessions, a third counts youth-days. Set it once per grant and the same session reports correctly to each.

Staff ratio and screening, tracked as you go

Ratio is calculated from who was actually in the room. Background screening dates sit on the staff record, and a lapsed re-screen is flagged before a monitor finds it.

Participant files, checked not assumed

Consent, release, intake, progress notes, discharge. Each youth shows what is on file and what is missing, with the document attached.

Signatures either way

Sign on the screen, or print a sheet with each youth's name, ID and a signature line, then scan it back. The packet keeps whichever you used.

Shortfalls surface early

If a period is tracking below a contracted minimum, you see it while there is still time to run more sessions, not in a corrective action letter.
